I confirm I will provide a copy of my identity document to the courier upon receipt of the order. Should a 3rd party receive my delivery I will ensure my ID, as well as the 3rd parties ID, is available at the point of delivery. I confirm that in order to receive my TV I must provide a valid TV licence. Should no TV licence be presented within the allocated 5-day timeframe your order will be cancelled. Should you not be available to receive delivery of your TV after 3 delivery attempts your order will be cancelled. I have read, understood and accept these terms and conditions mentioned above. By signing the POD I will confirm that I have inspected the physical condition of the packaging and the contents within. I, therefore I will declare that the order was delivered in a good condition.

All the information I have provided on this form is true and correct. I confirm having provided a copy of my identity document to the courier upon receipt of the order.
